Chronic disease self-management program to begin weekly sessions

LAKE PLACID - Adirondack Medical Center will host an evidence-based program developed at the Stanford Patient Education Center on Mondays, beginning March 9 through April 13. The program will run weekly from 1-3:30 p.m.

The chronic disease self-management program has techniques that will be taught through six participatory two-and-a-half-hour workshops. Some of the management topics to be addressed are pain, fatigue, anger, fear, frustration, stress, depression, nutrition, physical activity, relaxation techniques, improving communication, and planning for the future.

Anyone age 18 and older with a chronic condition or caring for someone with a chronic condition is encouraged to register. For a limited time, through grant support, the program is being offered at no cost to participants and comes with a free book, "Living a Healthy Life with Chronic Conditions."

Registration capacity per program is 10-15 people.
